In case of missing packing slip use the Manual Packing and Receiving Equipment form (pdf)
Equipment purchasing forms
Equipment costing > $25,000 use the Supplier Selection Justification form (pdf)
Equipment costing $100,000 or more, use the Equipment Screening Certificate form (pdf)
